This Notice describes how medical and mental health information about you may be used and disclosed by Therapy at NEST and your clinicians, and how you can get access to this information. Please review it carefully.
Our Commitment to Your Privacy
We are required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information (PHI) and to provide you with this Notice of our legal duties and privacy practices. We are committed to protecting your PHI and will use and disclose it only as described in this Notice.
How We May Use and Disclose Your Health Information
We may use and disclose your PHI for treatment, payment, and health care operations. For example: to provide, coordinate, or manage your care; to obtain payment from you, your insurer, or another party; and for our internal administration, quality improvement, and compliance activities. We may also disclose PHI as required by law, for public health activities, to avert a serious threat to health or safety, and in other limited circumstances permitted by HIPAA.
Uses and Disclosures Requiring Your Authorization
Other uses and disclosures of your PHI will be made only with your written authorization. You may revoke an authorization in writing at any time; we will honor the revocation except to the extent we have already relied on it.
Your Rights Regarding Your Health Information
You have the right to: request access to and a copy of your PHI; request an amendment to your PHI; receive an accounting of certain disclosures of your PHI; request restrictions on certain uses and disclosures; request that we communicate with you by alternative means or at alternative locations; and obtain a paper copy of this Notice upon request. We will not retaliate against you for exercising any of these rights.
Breach Notification
If a breach of your unsecured PHI occurs, we will notify you as required by law.
Changes to This Notice
We may change this Notice from time to time. The “Last updated” date at the top reflects the most recent version. A copy of the current Notice will be posted on our website and available upon request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
